Heath Ledger was a part of creating the Joker's makeup look.

The Talented Heath Ledger

Heath Ledger collaborated closely with the makeup team to develop the Joker's appearance in The Dark Knight.

He was "very involved" with the painting of his face, working alongside prosthetic supervisor Conor O'Sullivan and makeup artist John Caglione Jr. Ledger would scrunch his face during makeup application to create the character's worn and weathered look.

Additionally, he personally applied the Joker's lipstick, believing it was essential to his characterization.

Director Christopher Nolan also noted that the team gave a "Francis Bacon spin" to the Joker's face, aiming for a sense of corruption and decay in the texture of his look.

So, while Heath Ledger didn't design 100% of the makeup by himself, he played a significant role in shaping the Joker's final appearance.
